How a care plan is developed
Support is selected after assessing the personʼs condition, needs and the effect of symptoms on everyday life. A plan may include psychotherapy, guidance on sleep and daily routines, support from trusted people and regular professional follow-up. The combination and intensity of care depend on the individual situation.
Goals and expected outcomes are discussed at the outset, then progress is reviewed over time. If an approach is not helping enough or unwanted effects appear, the plan can be reconsidered with the professional. Do not stop agreed care or change a doctorʼs recommendations on your own.
